The level could be given editing their profiles after they create the accounts or, if he has s2Member Pro, use a free registration pro-form for Level 4 that he'd give its URL only to his family members.

He could, though, have their accounts be at Level 4 and since access is incremental for levels, they'd get access to the content of the paying members at lower levels.

If it's the first, you could just make an account under Dashboard -> Users -> Add User that has Administrative access.

If it's the second, then what you can do, is create a custom capability that gives access to everything as a free user, but only give out the link to your family.
