If you're using the pro-forms, then you can create custom registration fields and these will show in the pro-form. WP Admin -> s2Member -> General Options -> Custom Registration

You could also customzie the pro-form's template, if you were talking about a graphical customization. WP Admin -> s2Member -> PayPal Pro-Forms -> Shortcode Attributes -> template

Except for the Free Registration form, the Pro forms are really only for use with a PayPal Pro account.
You can use a pro form and a standard PayPal account if the buyers are specifically paying with PayPal but any other Pro Form use requires a PayPal Pro account.
