Cannot create Custom Registration Fields

I'm trying to add some custom registration fields, but I'm getting 2 very strange errors. I ONLY have buddypress activated, no other plugins. Running the latest version of WP and s2Member. Here are my errors:

After filling out my first custom reg field, I click the "Create Registration Field" button and nothing happens. Firebug reveals an error immediately upon the button being clicked. File is attached.

If I clicked the button a 2nd time, a javascript modal error pops up, it is also attached
