The odd thing is that I can only recreate this error in chrome and firefox. Internet explorer on my computer actually works (however, it does not work on my users computer who is trying to upgrade).

Any ideas on what might be causing this issue?
*Update 12.2.11 - I needed to get this user signed up yesterday, so I tested using my dummy account with firefox to fill in all profile data exactly as it would be for my dummy profile. The payment was attributed to my profile as it should. So, it looks like the it is recognizing the profile, just not populating the data. Unfortunately, the customer didn't sign-up, so I'll report back if another customer upgrades and has the same issue before a fix is created.
