Single registration payment form for multiple access levels

My client is stuck on providing a single registration form for all of our different access levels (like this one: http://www.rocktheshotforum.com/amember/signup.php)
Is there easy code I could use to do this (radio buttons or drop-down menu that decides which amount to charge)?
