Pro Forms: Missing Coupon Code but Card Issue Date showing

We are converting to Paypal PRO forms to offer coupon codes but the Coupon Code field is not showing on the checkout form. Also, the Card Issue Date is showing even though we only select Paypal, Visa and Mastercard.

(Version 111105 + s2Member Pro v111105)

The form shortcode is as follows:

Any pointers would be much appreciated.

Have you configured everything under WP Admin -> s2Member -> Pro ( Coupon Codes )?

Yes - the WP Admin -> s2Member -> Pro ( Coupon Codes ) setup is as follows:
SAVE30|30%
SAVE10|10.00

I'm also curious about the Card Issue Date that is showing though - almost as if the form is not generated correctly. I have also run the check_my_server.php file and all returns were successful.

Is there perhaps anything else that I can check?

You may be having a JavaScript conflict with the theme or another plugin.

Could you try as a test, changing the theme to TwentyEleven and see if the pro-form works properly then? If it still doesn't work, could you deactivate other plugins one-by-one and see if the problem goes away with one of them?
