Authorize.net billing modification and account cancel issue

I'm running S2Member Pro (authorize.net) in sandbox test mode. I set up a screen to allow users to change profile info and I also inserted the billing update and account cancellation shortcodes (copied directly from the Authorize.net Forms).

There are two issues on the screen
1 - In the billing section, the field for "Card Start Date" appears - I know this is an international field and should not appear for US
2 - An error message that says "This will cancel your account, are you sure?" appears above the Cancel Account field when the screen is loaded, before any button is clicked.

Is this happening because I'm in sandbox mode or are these bugs? or am I setting this up incorrectly?

I've seen a similar problem happen with the PayPal pro-forms, so it could be for the same reason here: a JavaScript conflict.

Could you try as a test, changing the theme to the default TwentyEleven or Ten, and if that doesn't solve it, deactivate other plugins one by one until the problem goes with it. Once you spot what's causing the problem, you can look into asking the developer to fix it or you can find a replacement.
