Login on separate page

Hi guys,
Using the s2 Pro, paypal form, I am offering free membership with single opt in setting, synced with Mailchimp.
I use a theme with disabled sidebars so I cannot use the login-widget. Instead I want to add the login option within a page.
(I do not want the opted-in user to visit the typical wp-login.php because I want the user to remain on the front end side of the site. Also, because it's single opt-in, no password or confirmation is sent via email. So once the user opts in, he needs to be sent to a page that says "thanks, now put in the email and password you just entered to get access to the area" with a box underneath it that has the form.)
Can you guys help me? I am a newbie and I can barely get wordpress going. Can someone please tell me what exactly I need to input in the html editor so the login "username" and "password" shows up with editable width and hight?
